New Patient?

We are currently accepting new patients and kindly request that those interested in establishing care with us contact our office by phone to confirm that your insurance will be accepted. To facilitate a smoother check-in process on the day of your appointment, new patients are encouraged to complete our online forms in advance. These forms can be sent via text through our automated system or downloaded from the link located in the lower right corner of this page. We understand that not all patients have access to online forms, and that is perfectly acceptable. If you have not completed the forms online, please arrive 15 minutes prior to your scheduled appointment. Additionally, if you have visited another healthcare provider within the past year before establishing with us, we recommend that you contact your previous provider to request that all relevant dental records be sent to our email address.
